Submission

Starting June 23, 2025, medical examiners must submit MEC information electronically to the National Registry of Certified Medical Examiners (NRCME), which transmits it to state licensing agencies within 72 hours. Drivers no longer need to submit paper copies to the state after this date but should carry a hard copy of the MEC

Blood Pressure

The FMCSA provides specific certification guidelines based on blood pressure readings:​

Stage 1 (BP 140-159/90-99): Eligible for a 1-year certification.

Stage 2 (BP 160-179/100-109): May receive a one-time 3-month certification; if BP is reduced below 140/90 within this period, a 1-year certification can be issued.

Stage 3 (BP ≥180/110): Disqualified until BP is controlled below 140/90; upon control, may be certified at 6-month intervals.

Exemptions and Waivers

Vision and Physical Impairments: Drivers not meeting vision or limb standards may qualify with a Vision Evaluation Report or SPE certificate. Intrastate Waivers: Some states, like Massachusetts, offer intrastate waivers for certain conditions, allowing drivers to operate within state lines. Diabetes: ITDM drivers can drive interstate with proper documentation and management.
