This page is for those who forgot to submit their medical card to the DMV prior to 5/23/2025.
On June 23rd, 2025, the Law Changed, and now the Medical Examiner must submit your Medical Card to the FMCSA (Federal Motor Carrier Safety Admin).
If you forgot to do this, I will re-submit your Medical Card to the FMCSA, and then the FMCSA will update it to Your DMV (this process takes 2-3 business days).
I will also email you a PDF copy of your New Medical Card (mcsa 5876) and Long form (mcsa 5875).
